Prime Care Coordination is a care coordination organization (CCO) serving 8,900 members throughout 41 New York Counties. Prime Care supports individuals with intellectual and/or developmental disabilities, many of whom have multiple and complex needs, by developing individualized care plans focused on the individual’s personal goals and dreams.
Prime Care’s person-centered approach offers complete support for each person, identifying and coordinating the appropriate providers and services their members need to live their lives in the most meaningful way.
