Health Home Core Services are the foundation for Care Coordination services and are used to help each individual and their family reach their goals.
For each Health Home Core Service, your Care Manager will assist you in the following ways:
Comprehensive Care Management
- Help you create your Life Plan and help you get it started
- Help you update your Life Plan
- Review your current service needs, such as your providers and support services, your goals and your engagement activities
- Provide you with education and support for health, wellness and decision-making
Care Coordination and Health Promotion
- Provide you with education and support for health, wellness promotion and prevention programs
- Coordinate and arrange for additional needed services and ensure you have what you need to follow your treatment plan
Comprehensive Transitional Care
- Tell you, your family and your support team about networks with local practitioners and health facilities, including emergency rooms, hospitals and residential/rehabilitation settings
- Help you and your family prepare for life changes, as well as changes in needed services
Individual and Family/Advocate Support
- Educate you, your family and your support network about support and self-help resources to increase knowledge, engagement and independence, and to support your ability to take care of yourself
- Share information with you and your family about educational resources and support services
Referral to Community and Social Supports
- Advise you, your family and your support network of available community-based resources
- Your Care Manager will explain how they will help in managing referrals, access, engagement, follow-up and coordination
- Assist you with identifying additional community-based resources and supports
Use of Health Information Technology to Link Services
- Use Health Information Technology (HIT) to ensure all your care is being coordinated to best support you
Basic HCBS Care Management is a very minimal coordination option and does not include coordination of health, wellness, behavioral health and family services. It also does not include all of the Health Home Core Services listed above.
Prime Care Coordination does not authorize services. You will work in partnership with your Care Manager to identify supports and services based on your wants and needs. Your Care Manager will be your link to help coordinate these services. OPWDD will continue to authorize supports and services.
