Options and Benefits
Health Home Care Management is a new way to coordinate care, combining developmental disability services and supports with health, wellness and behavioral health services. This new way of coordinating care offers better support for people, including those with complex needs.
With Health Home Care Management, you will enjoy

· Better information sharing across service systems
· More flexible and comprehensive service planning
Basic HCBS Plan Support will also be provided by the CCO, but it will be a very minimal coordination option—less than the support you currently receive if you are enrolled in Medicaid Service Coordination—and it will not include coordination of health care or mental health services. You are less likely to keep your current service coordinator if you choose this option.
If you do not want to receive the more comprehensive care management that will be provided with Health Home Care Management, you can talk to your service coordinator about the option to receive Basic Home and Community Based Services (HCBS) Plan Support.
Prime Care Coordination does not authorize services. You will work in partnership with your Care Manager to identify supports and services based on your wants and needs. Your Care Manager will be your link to help coordinate these services. OPWDD will continue to authorize supports and services.
