At Prime Care Coordination, we believe that people with intellectual and developmental disabilities deserve the best, most-effective and thoughtful care possible.
Prime Care Coordination is pleased to announce its initial designation as a Care Coordination Organization/Health Home (CCO/HH) to serve Individuals with Intellectual and Developmental Disabilities. This designation, from New York State’s Office for People With Developmental Disabilities and Department of Health, allows Prime Care to serve individuals across Western, Central, and Northern New York, as well as the Southern Tier. Prime Care is committed to providing individualized services and supports in geographic locations situated close to those who need them.
Additionally, Prime Care Coordination is fully committed to building a successful Care Coordination Organization by assembling a member network, building an experienced board, and working with the Office for People With Developmental Disabilities (OPWDD).
We’ve created a team of agencies and organizations that have a long history of providing high-quality care and support for people of all ages in urban, suburban and rural communities. Each agency on the Prime Care team is committed to helping people with intellectual and developmental disabilities and their families live fuller, richer and healthier lives.
